Dr. Kligman has admitting privileges at Northwest Medical Center. Inpatient needs are met

by the hospitalist system and Dr. Kligman often makes social visits when his patients

are admitted to the hospital.

When his office is closed, Dr. Kligman encourages patients to contact him via cell phone

520-405-5696 for minor needs or to proceed to an urgent care center if unable to wait for

his next available appointment. For urgent and life threatening needs, patients should

proceed to their closest emergency room.

NEW PATIENTS

Dr. Kligman looks forward to becoming your primary care physician and

expanding your primary care services through the AgeWell Longevity Program.

Currently Dr. Kligman is accepting the following insurance plans in addition to

cash payment for new patients:

* HealthNet

* Pacificare

* United Healthcare

* Medicare

If your insurance plan is not listed above, our office will be happy to provide you with a

superbill which can be submitted by you to your insurance company for reimbursement

of your cash payment at the time of service.

CURRENT PATIENTS

Dr. Kligman looks forward to continuing to assist you in reaching your optimal health and

vitality and will continue to accept your current insurance plan as previously arranged.

VISIT FEES FOR CASH PAY ONLY PATIENTS

New Patient Visit45 minutes $175.00One time fee.

Annual Health 40 minutes $155.00Year 2 onward.

Assessment

Follow-Up Visits 25 minutes $99.00 Scheduled as needed.

Yearly Physical 30 minutes $150.00 Additional fee for OBGYN visit.

REQUEST A PRESCRIPTION REFILLBack to top

* If you need a prescription refill, we encourage you to call your pharmacy directly

and they will fax us a refill request we will respond to within 24 hours.

* Whenever you need a refill, we strongly recommend you initiate a refill request

at least 48 to 72 hours before your current prescription ends this is especially

important when weekends and holidays are involved.

* Dr. Kligman will NOT approve routine refills on weekends or holidays, unless

they are urgent (eg, life threatening if the medication is not refilled).

* Routine pain medication will NOT be refilled after hours, since Dr. Kligman

does not have access to patient charts out of the office.
